About The Position

The Project Manager is responsible for planning, coordinating, and delivering projects across the functions of the Practice Enablement organization. This role turns strategic priorities into structured, well-run initiatives with clear scope, timelines, and outcomes; keeps cross-functional work on track; surfaces risks early; and ensures that commitments made to stakeholders are delivered as promised. The role requires strong follow-through and the ability to build effective working relationships across teams, prioritize competing demands, and adjust plans as priorities shift while incorporating change management considerations to support successful adoption of new processes and tools. The ideal candidate bridges strategy and execution — this is a hands-on delivery role focused on the execution of Home Health Practice strategic priorities and initiatives and does not own the practice operating model, which sits within Operational Excellence.

Requirements

  • Bachelor’s degree in business, Project Management, Healthcare Administration, or related field, or equivalent experience.
  • 2+ years of demonstrated project management experience leading cross-functional initiatives from start to finish.
  • Proven ability to build and manage project plans, timelines, milestones, and status reporting.
  • Familiarity with change management principles and practices, and the ability to incorporate change management considerations into project plans.
  • Track record of delivering projects on time while coordinating multiple stakeholders and workstreams simultaneously.
  • Ability to prioritize competing tasks and adjust plans as demands shift across stakeholders.
  • Strong stakeholder coordination, follow-through and accountability skills with the ability to build effective, collaborative working relationships across teams and functions.
  • Excellent written and verbal communication, with the ability to tailor updates for both working teams and senior leadership.
  • Strong organizational skills, attention to detail, and the flexibility and adaptability to adjust plans as priorities or organizational demands shift.

Nice To Haves

  • Formal project management certification (e.g., PMP, CAPM) preferred but not required; demonstrated hands-on experience will be considered equivalent.
  • Experience gathering and documenting business requirements, or exposure to business analysis practices, preferred.
  • Experience in home health, healthcare, or distributed field-operations environments preferred.
  • Familiarity with AI Tools (e.g.,Claude, Copilot) to support project efficiency and productivity preferred.

Responsibilities

  • Develop working knowledge of BAYADA’s mission, services, people, organization, policies and procedures.
  • Lead the full lifecycle of assigned projects — intake, planning, execution, monitoring, and closeout — across the functions of the Practice Enablement organization.
  • Build and maintain project plans, timelines, milestones, and status reporting that give leadership clear visibility into progress and risk.
  • Coordinate cross-functional teams and stakeholders, holding owners accountable to commitments and removing blockers.
  • Identify, track, and escalate risks, issues, and dependencies before they affect delivery.
  • Facilitate project meetings, working sessions, and stakeholder updates; document decisions and action items.
  • Partner with function leads to translate strategic goals into scoped, resourced, and sequenced work.
  • Incorporate change management considerations into project plans and execution to support successful adoption of new processes and tools.
  • Leverage AI tools to improve efficiency in project tracking, reporting, and documentation.
  • Support portfolio-level coordination so that concurrent initiatives are prioritized and sequenced appropriately.
  • Utilize current project management methodologies, tools and templates as well as continuously improve the project management practices, tools, and templates used across the organization.
  • Demonstrate solid performance or exceed performance standards in key job dimensions/attributes as defined on the Performance Appraisal for Office Staff.
  • Perform related duties, or as required or requested by supervisor.
