As Project Manager II, you will manage small projects or phases of larger projects, coordinating project team activities under the direction of a Project Manager III or IV. In this role, you will support the planning, organization, execution, and evaluation of project activities to achieve successful outcomes. The role focuses on implementing value-based strategy initiatives and population health programs designed to improve patient outcomes and help patients remain healthy and at home longer. You will work cross-functionally with enterprise partners, including primary and specialty physicians, pharmacy, nursing, social work, and other caregivers, with a primary focus on Medicare Advantage and ACO populations and priority contracts. This position will support initiatives such as implementing technology solutions, developing and improving clinical workflows like discharge planning and scheduling, creating QRGs and patient documentation, and coordinating delegated materials. As part of these efforts, you will facilitate meetings, document milestones, coordinate project activities, identify barriers, and help keep projects moving forward. A caregiver in this role works remotely from 8:00 a.m. – 5:00 p.m., with an anticipated on-site requirement of 1–2 days per week at either the CCAC or Independence FHC location.

Requirements

  • Bachelor’s Degree in Business or a related field and three years of experience coordinating or managing various projects OR High School Diploma/GED and seven years of experience OR Associate’s Degree and five years of experience OR Master’s Degree and two years of experience
  • Effective relationship building skills
