Both CHNw and DHS place high importance on continuing and advancing population health and value-based care to improve patient health outcomes. Make a Difference The Innovative Healthcare Collaborative of Indiana LLC (IHCI) is seeking an organized and collaborative Project Coordinator to support Population Health Operations initiatives. Reporting to the Director of Population Health Operations, this role will coordinate projects, facilitate meetings, maintain project documentation, track progress, and support process improvement efforts across the organization.

Requirements

  • A bachelor's degree in a related field is required.
  • Two (2) years of project coordination or related experience, preferably in healthcare or process improvement environments.
  • Two (2) years of experience managing multiple priorities in a fast-paced setting.
  • Strong organizational, communication, and stakeholder engagement skills.
  • Experience with Microsoft Office applications, including Excel, PowerPoint, and Teams.
  • Working knowledge of project management methodologies such as Agile, Scrum, Lean, or Waterfall.
  • Ability to build relationships across all levels of the organization.
  • Self-motivated, adaptable, and comfortable managing multiple projects simultaneously.
  • Passionate about improving processes and supporting organizational initiatives.

Nice To Haves

  • Project Management Professional (PMP), Lean Six Sigma, Scrum Master, Agile Certified Practitioner, or similar certification preferred.
  • Healthcare, population health, or operational improvement experience preferred.

Responsibilities

  • Coordinate project plans, timelines, deliverables, and documentation for population health and process improvement initiatives.
  • Facilitate project meetings, develop agendas, capture meeting notes, and track action items.
  • Monitor project progress, risks, issues, and milestones while providing regular status updates to stakeholders.
  • Coordinate and manage executive leadership calendars to support project update meetings and related activities.
  • Collaborate with leaders, vendors, and cross-functional teams to support project success.
  • Assist with reporting, presentations, metrics tracking, and project logistics.
  • Support data-driven improvements that enhance population health results and operational effectiveness.
