A Brief Overview The Project Austin Outreach Liaison serves the medically complex patient population at Children's Nebraska and is a core team member to the continuity of patient care planning. The Outreach Liaison acts in an expanded health care role to collaborate across the continuum with the patient's Children's medical team, caregiver, local Emergency Medical System and local Emergency Department to ensure the delivery of quality, cost-effective health care services. The Outreach Liaison assesses plans, implements, coordinates, monitors, and evaluates all options with the goal of optimizing the patient's health status. The Outreach Liaison integrates evidence-based clinical guidelines, preventive guidelines, and protocols in the development of individualized emergency care plans that are patient-centered, promoting quality and efficiency in the delivery of health care across the continuum. Communicates effectively amongst diverse patient populations and providers. In partnership with the patient's medical health team, the Outreach Liaison leads emergency care coordination within the team through process improvement, workflow redesign, aiding with training, and delegating to other members of the team.
