About The Position

The Project Austin Outreach Liaison serves the medically complex patient population at Children's Nebraska and is a core team member to the continuity of patient care planning. The Outreach Liaison acts in an expanded health care role to collaborate across the continuum with the patient's Children's medical team, caregiver, local Emergency Medical System and local Emergency Department to ensure the delivery of quality, cost-effective health care services. The Outreach Liaison assesses plans, implements, coordinates, monitors, and evaluates all options with the goal of optimizing the patient's health status. The Outreach Liaison integrates evidence-based clinical guidelines, preventive guidelines, and protocols in the development of individualized emergency care plans that are patient-centered, promoting quality and efficiency in the delivery of health care across the continuum. Communicates effectively amongst diverse patient populations and providers. In partnership with the patient's medical health team, the Outreach Liaison leads emergency care coordination within the team through process improvement, workflow redesign, aiding with training, and delegating to other members of the team.

Requirements

  • Bachelor's Degree From an accredited college or university in nursing Required
  • Minimum 5 years in nursing in a pediatric setting required
  • RN - Registered Nurse - State Licensure and/or Compact State Licensure Current and valid Nebraska RN license/RN Compact license Required
  • BCLS - Basic Life Support through the American Heart Association Required
  • PALS - Pediatric Advanced Life Support Required
  • Local, regional, and statewide travel for community training and outreach events Required
  • Evening and weekend hours for training and outreach events Required
  • Demonstrated written and verbal communication skills.
  • Ability to apply the nursing process to patient care to achieve outcomes.
  • Current knowledge of the healthcare environment, including community resources.
  • Working knowledge of basic computer programs such as Word and Excel

Nice To Haves

  • Minimum 1 year experience in outreach, emergency medical services or transport preferred
  • CPN - Certified Pediatric Nurse Preferred

Responsibilities

  • Directs and Oversees Project Austin Education and Outreach Events
  • Serves as a role model, educator, consultant, and researcher around emergency care of the medically complex pediatric patient population
  • Oversees the quality of emergency care of the medically complex pediatric patient population through outreach to the community and region by interacting with and assisting in the utilization of available resources of emergency medical services (EMS) agencies and Emergency Departments (ED)
  • Acts as a liaison for identification of the medically complex pediatric population related education activities and outreach events
  • Continually monitors EMS and ED utilization of emergency care plan as indicated
  • Completes timely follow-up activities with EMS and ED
  • Presents or coordinates collaborative community training events with patients, families, referral hospital and emergency medical services
  • Completes annual Pedi Pack requests and active patient lists for referral hospitals and emergency medical services
  • Creates and maintains up-to-date, evidence-based education for EMS and hospitals
  • Collaborates with Providers, patients, families, and members of health care teams across a continuum of care settings and the community.
  • Fosters a team approach and includes patient/family as active members of the team.
  • Takes the lead in ensuring the continuity of care which extends beyond the hospital boundaries.
  • Serves as liaison to specialists, post-acute care services and community agencies including EMS and ED.
  • Maintains comprehensive medical record documentation on the appropriate information systems and databases using required documentation and billing guidelines.
  • Utilize her activities to manage enrollment and maintenance for patients, EMS, and ED
  • Assist in the development of adequate workflows to ensure patient care, EMS and ED needs are met.
  • Proactively reach out to EMS and ED to maintain up-to-date liaison contact information
  • Facilitate the use of bulk ordering and communications with groups of patients, EMS and ED
  • Manage the care of high-risk patients using EHR patient outreach tools.
  • Participates in teaching and coaching clinic staff in the management of patient conditions. Assesses staff educational needs and develops educational programs. Promotes opportunities and activities.
  • Works with office and leadership to continuously evaluate process, identify problems, and propose/develop process improvement strategies to enhance care management and the Patient Centered Medical Home delivery of care model.
  • Provides support to RN Case Manager – Project Austin.
