About The Position

This role is responsible for facilitating day-to-day programming options for individuals, ensuring these options are person-centered, age-appropriate, and community-focused. The coordinator acts as the designated programming lead for the center, developing programs based on the interests of the individuals served. This position involves working cooperatively with other staff and community employers to ensure successful outcomes for the individuals. Key duties include developing, recording, and implementing various support plans, maintaining up-to-date client files and databases, and reviewing medical needs with the support team. The role also involves assisting individuals with personal needs and adhering to all agency policies and procedures, including Rule 245D standards.

Requirements

  • Strong verbal, writing, and math skills.
  • Proficiency in basic computer skills, including word processing and spreadsheets (Word, Excel, Outlook).
  • Ability to work both independently and cooperatively with coworkers and supervisors.
  • Ability to relate well with people with disabilities.
  • Ability to lift and transfer adults with physical disabilities, using individualized methods.
  • Ability to frequently bend, stoop, reach, and push manual wheelchairs.
  • Must pass background checks as required by the MN Department of Human Services, MSS, and applicable law.
  • Must secure/maintain certification in First Aid, CPR, and Medication Administration.
  • Must have a valid driver’s license and be insurable under the standards set by our insurance carrier.

Responsibilities

  • Facilitate day-to-day programming options for persons served, ensuring they are person-centered, age-appropriate, and community-focused.
  • Develop programming within the center based on the interests of individuals served.
  • Develop, record, and implement CSSP-A’s, SMA’s, IAPPs, ISP's and other plans in cooperation with the Support Team.
  • Maintain up-to-date files for assigned individuals, including the MSS Client Database, and ensure other program records are completed accurately and timely.
  • Review the medical needs of assigned individuals with the Support Team and Nurse Consultant.
  • Assist persons served with their personal needs as necessary, including using the bathroom, eating, and dressing.
  • Abide by and implement all agency policies and procedures, including Rule 245D standards, Data Privacy, Emergency Use of Manual Restraints, and VA Policy and Procedure.
  • Communicate program plans and medical needs to Direct Support Professionals and other direct service staff.
  • Provide guidance and instruction to direct service staff on the implementation of plans and procedures.
  • Instruct direct service staff in day-to-day expectations for maintaining daily documentation.
  • Plan and implement person-centered individualized and group activities within the center and in the community in cooperation with the Program Director, Program Supervisor, and other Service Coordinators.
  • Assume duties of other Service Coordinators in their absence as directed by the Program Director.
  • Secure and maintain medication certification and be responsible for all aspects of medications as assigned.
  • Secure and maintain CPR and First Aid certifications and provide emergency care when needed.
  • Act as a liaison between MSS and residential providers in transportation concerns for persons served.
  • Ensure that MSS drivers and contracted drivers have the necessary emergency information for persons served.
  • Ensure the safety of persons served and coworkers by following all safety-related policies and procedures.
  • Promptly report any unsafe conditions to the Program Director.
