About The Position

The IHS/Adult Companion Program Team Lead oversees individualized supports for adults with developmental disabilities living independently or in community settings. This role provides person-centered planning, skill development, case management, and resource coordination while ensuring services promote independence, community inclusion, and alignment with KDC’s mission and regulatory standards. The Team Lead manages daily program operations, supervises Adult Companion contractors, supports recruitment and onboarding, and organizes community engagement opportunities. This role works collaboratively with the Individual Supports Director using a multidisciplinary approach aligned with Kennedy-Donovan Center’s (KDC) philosophy, mission, and vision.

Requirements

  • Minimum of three years of recent professional health-care experience working with elders or adults.
  • A valid driver’s license and reliable access to a vehicle.
  • Excellent organizational, interpersonal and communication skills.
  • Experience in providing home-based supports.
  • Commitment to community-based supports to individuals with developmental disabilities.
  • Commitment to family-based services.
  • Ability to work independently while maintaining strong teamwork and partnership with caregivers and community providers.
  • CPR/First Aid certification (or ability to obtain within required timeframe).
  • Proficiency with Microsoft Office and electronic healthcare records.

Nice To Haves

  • Bachelor’s degree in Social Work, Psychology, Human Services, Healthcare Administration, or a related field.
  • Supervisory or team leadership experience in a healthcare or social services environment.
  • Knowledge of state regulations related to DDS (Department of Developmental Services) and other funding agencies.
  • Experience with electronic healthcare records systems such as HCSIS and Icentrix.
  • Bilingual skills (e.g., Spanish, Portuguese, or other languages commonly spoken in our service area) highly desirable.
  • Certification in crisis intervention, behavioral support, or relevant training related to developmental disabilities.

Responsibilities

  • Provide skill training, case management, supportive counseling, and crisis intervention to IHS participants.
  • Participate in the development and review of Individual Support Plans (ISPs) to support health, safety, and personal goals.
  • Conduct regular visits to assess safety, well‑being, and satisfaction.
  • Facilitate opportunities for independence, social connections, employment, and recreational activities.
  • Connect individuals with community services, supports, and activities.
  • Provide transportation to appointments, activities, and meetings as needed.
  • Work with healthcare providers, community agencies, families, and funding sources.
  • Maintain accurate, timely documentation in accordance with KDC policies, DDS requirements, and funding standards (including HCSIS, Icentrix, case notes, and individual files).
  • Monitor program operations to ensure compliance with state, federal, and agency regulations.
  • Oversee Adult Companion contractors to ensure quality service and adherence to DDS standards.
  • Lead recruitment, onboarding, and training for Adult Companions.
  • Identify and facilitate staff development opportunities in partnership with the Program Director.
  • Develop, manage, and monitor expenditures to ensure responsible use of resources.
  • Identify and organize community events that enhance inclusion and engagement.
  • Participate in the on‑call rotation to provide after‑hours support and crisis response.
  • Represent KDC at agency, legislative, and industry events.
  • Perform other duties as assigned.
