About The Position

Carefirst Seniors & Community Services Association is a non-profit charitable community services agency established since 1976. Our spectrum of services has grown from the delivery of Chinese meals-on-wheels service to a full range of community support services, covering the whole Greater Toronto Area. With a staff of 500 members (including full-time and part-time staff) and the help of a pool of 850 active volunteers, we serve over 15,000 clients a year, including seniors, adults, youth with physical, social, emotional and health challenges. 3,000 of whom are "home-bound" and frail. We are currently seeking a Team Lead / Program Manager to fill a vacant position at our Community Support Services team.

Requirements

  • Master/Bachelor of Social Work from an accredited institution
  • Current registration in good standing with the Ontario College of Social Workers
  • Minimum of 5 years of frontline experience in social work intervention in a community-based setting
  • Minimum of 2 years of experience in a supervisory or leadership capacity within social services/health care sector
  • Proficient in case management, coordination, and psychological counselling techniques
  • Strong understanding of social support services, welfare system and community resources in the GTA
  • Excellent client service skills including needs assessment, care plan development, and group facilitation
  • Knowledge of community resources for health promotion, chronic disease management, and prevention
  • Experience in developing, implementing, monitoring and evaluating education programs and activities within communities
  • Strong communication, time management and problem solving skills
  • Ability to build positive relationships with clients, families, volunteers, and community stakeholders
  • Proficiency with Microsoft Office applications and other data-based applications (AlayaCare) is an asset
  • Fluency in written and spoken English, a second language commonly used by clients, i.e. Cantonese or Mandarin, is preferred
  • Valid vulnerable sector police record check completed within the past 12 months
  • Valid driver's license and access to a reliable vehicle is an asset

Responsibilities

  • Provide direct client services, including intake assessments, care plan development, case management, client interventions, support counseling, service navigation, and providing information and referral services
  • Deliver and facilitate group activities, education workshops, special events and projects to address the needs of the seniors/caregivers/families
  • Advocate for clients/caregivers/families on social, psychosocial and mental health matters, ensuring culturally sensitivity
  • Assess community needs and strengths, identify gaps in service delivery and propose innovative solutions or new program models to address emerging needs in the aging population
  • Support the strategic planning, coordination and evaluation of the Community Support Services Team to optimize service delivery.
  • Assist in building and maintaining strategic partnership with local healthcare providers, government agencies and other service organizations to enhance the referral network and resources pool
  • Provide clinical/administrative supervision to staff and students and support internal training for staff on topics relating to aging in community, dementia care, elder abuse prevention, and system navigation
  • Maintain accurate and up-to-date records, including cases files, group activities, program statistics, evaluation reports, and financial documents for fee-based services
