The state supported living centers serve people with intellectual and developmental disabilities who are medically fragile or who have behavioral problems. The centers provide campus-based direct services and supports at 13 centers located throughout the state. The Post Move Monitor (PMM) works under the general supervision of the Admissions/Placement Coordinator. Work involves performing routine consultative services and providing technical assistance during monitoring of individuals following their move from a State Supported Living Center (SSLC) to a community setting. The PMM assists in the development of supports and services as part of the transitioning planning process. Observes and interacts with individuals and community provider staff to determine if the supports and services identified in the transition plan are in place and effective for the individual. The PMM completes a written report and notifies SSLC staff of any identified issues. Mediates disputes/dissatisfaction with community supports and services following transition from the facility and works to develop solutions for identified issues. Works under moderate supervision, with limited latitude for the use of initiative and independent judgment. This position is required to travel up to 80% of the time.

Requirements

  • Ability to prepare reports and communicate effectively orally and in writing.
  • Ability to work professionally with individuals with intellectual disabilities, family members, staff, community providers, and others.
  • Knowledge of Health and Human Services Commission (HHSC) community programs including ICF/IID and waiver programs.
  • Ability to gather, assemble, correlate, and analyze facts and to devise solutions to problems.
  • Computer skills in WORD, EXCEL, and other Microsoft Office products.
  • Able to effectively handle multiple responsibilities and plan work activities.
  • Ability to travel up to 80% of the time.

Nice To Haves

  • Experience working in community Immediate Care Facilities/Intellectual Disability (ICF/IID) or Home Community-based Services (HCS) waiver programs is preferred.
  • Experience working with individuals with intellectual disabilities, or physical disabilities is preferred.
  • Willingness to travel 80% of the time.

Responsibilities

  • Attends work on a regular basis and may be required to work a specific shift schedule or, at times, even a rotating schedule, extended shift and/or overtime in accordance with agency leave policy and performs other duties as assigned. May serve in an on-call status on a rotating schedule as required to meet operational needs of the facility.
  • Provides support and collaboration in the planning and development of community supports as part of the transition planning process for persons moving from the SSLC to a community setting.
  • Schedules, coordinates, and conducts on-site monitoring of individuals following their move from the SSLC to a community setting. Monitors the presence of supports and delivery of services outlined in the Community Living Discharge Plans (CLDP) for each individual. Observes and interacts with individuals in both their home and day program settings at 7, 45, and 90 intervals and more often as needed.
  • Completes monitoring checklists following each visit and communicates results of the visit with the Admission/Placement Coordinator and the interdisciplinary team as needed to address identified issues.
  • Mediates disputes/dissatisfaction with community supports and services following transition and works to develop solutions for any identified issues.
  • Other duties as assigned include but are not limited to actively participating and/or serving in a supporting role to meet the agency’s obligations for disaster response and/or recovery or Continuity of Operations (COOP) activation. Such participation may require an alternate shift pattern assignment and/or location.
