Requirements

  • Bachelor degree in a human service or related field or certification in a mental health specialty (e.g., RN, LVN, LPT, drug alcohol rehab counselor) preferred.
  • Minimum 3 years of experience providing psychosocial rehabilitative services to a disabled population.
  • Experience in a supervisory/leadership role.
  • Demonstrated ability to provide supervision and leadership to diverse staff.
  • Must function well in a team-oriented approach to client services.
  • Proficient in computer applications such as Microsoft Word, Excel, and Outlook.
  • Ability to learn and utilize CHAMP, Homeless Management Information Systems (HMIS), and Electronic Health Records Systems.
  • Expertise in chronic homelessness, outreach and engagement strategies, intensive case management services, best practice models, mental health and substance use disorder services, crisis intervention, suicide assessment and prevention, affordable housing and public benefits applications, housing and landlord/tenant rights, and eviction prevention.
  • Ability to work collaboratively and effectively with public and private housing providers, mental health consumers and their family members or client advocates, vendors, and other staff.
  • Ability to track and interpret complex data sets for program services and budget.
  • Knowledge of community resources.
  • Flexible work schedule to include some nights, weekends, and holidays.
  • Driving with a current class C driver’s license, a vehicle for transportation, and proof of valid automobile insurance are conditions of employment.
  • Ability to be insured by MHALA liability insurance is a condition of employment.
  • TB clearance and re-testing every year are conditions of continued employment.

Responsibilities

  • Oversee the daily operation of Intensive Case Management Services (ICMS) for clients eligible for permanent housing through the Department of Health Services’ (DHS) Housing for Health (HFH) program, based on a psychosocial rehabilitation model.
  • Recruit, hire, supervise, train, direct, evaluate, reward, discipline, and terminate assigned staff in accordance with organizational policy and applicable laws and regulations.
  • Provide direct supervision and encourage professional growth for staff and interns, including those working in Lancaster, Palmdale, and Newhall offices.
  • Track, assess, and document program effectiveness in meeting objectives.
  • Manage the day-to-day activities of all Housing for Health (HFH) Project-Based and Scattered-Site housing, including HACola and HACLA Shelter Plus Care (SPC), staff supervision, team support, and coordination of Statements of Work.
  • Serve as a liaison with the Departments of Health Services and Mental Health.
  • Provide case management and/or mental health services in permanent supportive housing, including maintaining a crisis response system.
  • Provide timely information and feedback to the Assistant Director regarding staff, team, or participant issues and collaborate with executive management for interventions.
  • Assist in overseeing and managing team budget and expenditures, making necessary adjustments.
  • Support program documentation standards in accordance with MHALA, HACoLA, HACLA, LACDMH, DHS County Contracted Provider, and funding source requirements, including timely documentation in the Electronic Health Record (EHR).
  • Oversee and manage petty cash as custodian.
  • Assume or delegate leadership of the day-to-day operations of the team as needed.
  • Conduct team meetings to discuss client status, allocate staff work, and conduct case consultations.
  • Meet with the director of housing regularly.
  • Develop and implement quality control plans.
  • Collaborate with property management agencies and other supportive services agencies to ensure client needs are met.
  • Build collaborative partnerships with county funders, stakeholders, and community partners.
  • Develop policies and procedures and create forms for effective intensive case management in permanent housing.
  • Act as the point of contact for participant crises and emergent needs.
  • Participate in meetings with DMH, DHS, HACoLA, HACLA, and community partners like homeless outreach teams and Coordinated Entry System (CES) teams.
  • Engage in professional development activities to maintain and enhance skills.
