NewYork-Presbyterian (NYP) is a highly-ranked hospital committed to community health through its Ambulatory Care Network (ACN) and Division of Community and Population Health (DCPH). The Comprehensive Health Program (CHP) within this division is a center of excellence for various infectious diseases and addresses social determinants of health through comprehensive care coordination, multidisciplinary team-based care, integrated behavioral health, and primary care. Patients receive care from a team including care coordinators, social workers, RNs, and primary care providers, with Health Home enrollment offered as part of this comprehensive approach. NYS Health Home programs aim to manage complex medical conditions and address medical, behavioral, and social needs holistically, empowering patients in their healthcare journey. This Program Manager role will oversee care coordination programming and Health Home program activities. The position involves leading these programs alongside other Program Managers, managing day-to-day operations, monitoring program requirements (billing, documentation, data reports), and guiding the strategic vision for program sustainability. The role requires collaboration with team members, including direct supervision, coaching, guidance, and feedback to care coordinators. Strong organizational skills and the ability to interpret data reports for high-fidelity data, documentation, and billing are essential. Familiarity with grant-funded program implementation, evaluation, and management is beneficial, as the role will contribute to the enhancement of existing grant-funded programming. Effective supervision of care coordinators and excellent communication skills (verbal and written) are crucial. Experience in case management and/or behavioral health will aid in guiding care coordinators through clinical and administrative challenges. Program Managers will participate in the Care Coordination Steering committee meetings and collaborate on future program directions. Relationship building and networking skills are vital for collaborating with NYP departments and community organizations to ensure program success and visibility. The Program Manager will track program outcomes for reporting to funders, senior leadership, and other stakeholders.
