About The Position

NewYork-Presbyterian (NYP) is a highly-ranked hospital system committed to community health through its Ambulatory Care Network (ACN) and Division of Community and Population Health (DCPH). The Comprehensive Health Program (CHP) within this division focuses on HIV treatment, prevention, STIs, and infectious diseases, employing integrated care coordination strategies and multidisciplinary team-based care to address social determinants of health and improve outcomes for vulnerable populations. Patients receive care from a team including care coordinators, social workers, RNs, and primary care providers, with Health Home enrollment offered as part of this comprehensive approach. NYS Health Home programs aim to manage complex medical conditions and address medical, behavioral, and social needs holistically, empowering patients through collaborative, patient-centered care plans.

Requirements

  • Bachelor's Degree
  • Minimum of 5 years of experience
  • Minimum of 2 years supervisory experience
  • Strong written and oral communication skills
  • Experience in program development, implementation and evaluation

  • Master’s in social work/related clinical field or Master’s in Public Health
  • Experience in program management, development, implementation and evaluation
  • Interest in Quality Improvement strategies/planning
  • Familiarity with care coordination and program management including ensuring deliverables are met and reporting out to key stakeholders.
  • Comfort with data reports and tasking team members to achieve high quality outcomes for patients and meet programmatic requirements.
  • Familiarity with Grant Funded program implementation, evaluation, and management.

Responsibilities

  • Oversee care coordination programming and Health Home program activities.
  • Lead care coordination and Health Home programs, overseeing day-to-day operations.
  • Monitor all program requirements, including billing, documentation, and data reports.
  • Guide the implementation of the program's strategic vision for sustainability and longevity.
  • Collaborate with team members and provide supervision, coaching, guidance, and feedback to care coordinators.
  • Interpret and apply findings from data reports to ensure high fidelity of data for programmatic requirements, documentation, and billing.
  • Participate in the implementation and enhancement of existing grant-funded programming.
  • Supervise care coordinators effectively.
  • Communicate effectively verbally and in writing.
  • Guide care coordinators to resolve clinical and administrative challenges, leveraging case management and/or Behavioral Health experience.
  • Participate in the program’s Care Coordination Steering committee meetings and collaborate on future program directions.
  • Build relationships and network with NYP departments and community organizations.
  • Track program outcomes to report to funders, senior leadership, and other stakeholders.
