The Comprehensive Health Program provides an integrated model of care that includes primary medical care, specialty care and essential support services for people living with HIV or at risk for HIV, who are uninsured or underinsured. The CHP works with cities, states, and local community-based organizations to provide HIV care, treatment, and prevention services (Hep C, PrEP and PEP) to more than 5000 individuals each year. Grant funding is a key resource for the sustainability and diversity of our programming. This pivotal role will support robust grant programming for Care Coordination Services for People with HIV and Outpatient Ambulatory Services for People Aging with HIV. The role involves collaboration with staff throughout the NYP system and other community-based organizations in Manhattan, Brooklyn, and the Bronx.
