About The Position

Develop and oversee all required documentation for the client's Person-Centered Services. Ensure documentation meets requirements and timelines per the Developmental Disabilities Individual Budgeting Waiver Services Coverages & Limitations Handbook. standards. Ensure documentation is appropriately filed in clients’ central files. Promote and demonstrate The Arc's values, vision, and mission, ensuring the respect, dignity, safety, and well-being of clients at all times.

Requirements

  • Possession of a high school diploma or equivalent (GED).
  • Two years of experience in a related field.
  • College coursework in a related field may substitute for experience (30 credit hours is equivalent to one year of experience).
  • High level of practical verbal and written communication skills.
  • Strong interpersonal and organizational skills.
  • Ability to manage time effectively and prioritize.
  • Ability to work independently and in team settings.
  • Ability to promote the dignity and respect of those we serve.
  • Proficient with Microsoft Office and Google Suite software.
  • Proficient in operating essential office equipment such as copy machines, facsimiles, and computers.
  • Maintain positive interactions with clients, parents, guardians, co-workers, supervisors, and community members.

Responsibilities

  • Treat all clients, parents, guardians, co-workers, supervisors, and community members with respect and dignity.
  • Follow the policies, procedures, and rules of the organization.
  • Serve as a role model and demonstrate, encourage, and offer direction in fairness, consistency, attentiveness, ability to work with others, appropriate dress and appearance, and effective conflict resolution.
  • Work closely with supervisor to ensure implementation plans and reports.
  • Meet regulatory standards and fit the service environment.
  • Maintain HIPPA confidentiality of those we serve.
  • Attend Support Plan meetings for each client within the assigned caseload.
  • Meet with clients and direct support staff to develop Short Term Objectives (STO) and Strategies for Goal Tracking.
  • Create Implementation Plan, Goal/Activity Tracking Sheets, and Quarterly/Annual Summaries for each client within the assigned caseload according to the Support Plan date.
  • Complete Monthly and Quarterly/Annual Summaries using supporting data from Progress Notes, Goal/Activity Tracking Sheets, and/or meetings with the Treatment Team for each client within the assigned caseload.
  • Keep organized electronic records of completed Daily Notes, Goal Tracking Sheets, Monthly Summaries, and Quarterly Summaries for each client within the assigned caseload.
  • Provide hard copies of Progress Notes, Goal Tracking Sheets, Monthly Summaries, and Quarterly Summaries to the central file room for each client within the assigned caseload.
  • Revise clients’ Short Term Objectives and/or strategies as necessary.
  • Interact with clients within assigned caseload weekly.
  • Ensure all direct support staff are completing and updating the My Vision packets with clients.
  • Review Progress Notes weekly and Goal/Activity Tracking Sheets monthly.
  • Train direct support staff on correctly completing each data tracking sheet as necessary.
  • Be familiar with and refer to Qlarant Tools and i-Budget Handbook for documentation requirements.
  • Maintain high program standards.
  • Advocate for clients’ rights.
  • Other administrative duties as assigned.
