About The Position

Kahak Health Care Services is a service provider for individuals with intellectual and developmental disabilities. Our mission is to foster independence through compassionate support, helping individuals to achieve their goals and enhance their quality of life. We provide in-home support services, community coaching, and respite care. As an employee of Kahak Health Care Services (KHCS) you will be a part of a team of skilled and dedicated professionals who value integrity, respect, and teamwork. The Program Director oversees and ensures the smooth functioning of services through planning, monitoring, and evaluation to promote innovation, quality, and effectiveness of services.

Requirements

Effective communication both verbally and in writing
Planning, monitoring, evaluation of services to ensure each person’s needs are met
Ensure that all office equipment’s, staffing needs are met to ensure that the programs are running smoothly.
Evaluate programs and provide feedback as needed to the board
Supervise staff to ensure that each person is doing his or work geared towards meeting the needs of the coworkers and individuals and families we serve.
Attends necessary workshops, events and training seminars and updated policies, programs as needed
Ensures that all the staff have all the tools that they need for the programs to be fully implemented and to function smoothly.
Supervises Program Manager(s) to ensure compliance with state regulations and Kahak policies and procedures
Collaborating with Support Coordinators/Case Managers/CSWs of individuals on your caseload to ensure appropriate and effective provision of supports. This includes attending Individual Support Plan (ISP) meetings (in person or virtually), reporting incidents, and strategizing for the best care solutions of each individual.
Communicating to DSPs accurate policies, updates, and providing ISP and annual trainings
Foster and promote a person-centered approach to provision of support services in which the choices, dignity, and independence of each individual on your caseload is considered and respected
Demonstrates an understanding of community-based support models and relevant state regulations.
