Program Counselor (Case Manager)
About The Position

Our programs are grounded in a person-centered philosophy, meeting each individual where they are and supporting them toward greater independence at every stage of life. Vista is seeking a Program Counselor (Case Manager) to coordinate, monitor, and document counseling and case management services provided to Vista's Engage members. In this role, you will serve as a case manager and work closely with members, families, and staff to develop individualized goals, coordinate services, monitor progress, and support members in building greater independence and community involvement.
Requirements
- Bachelor’s degree plus one year of related experience.
- Valid driver’s license and maintain an acceptable driving record as defined by Vista.
- CPR, First Aid and PMT certification or the ability to obtain certifications within 90 days of hire.
- Ability to work independently, utilize good judgment, and demonstrate a strong sense of initiative.
- Transport students and members in either personal or Vista car/van to community sites in a variety of settings and weather conditions.
Responsibilities
- Develop written program plans (IPPs and IPs) for each member that evaluate progress, determine individual goals, and identify programmatic service needs.
- Implement systems that support each assigned member’s programmatic and individual goals in four main areas: social/emotional, vocational, life skills, and community involvement.
- Provide direct instruction in areas such as social skills/cognitive development, medication, money management, and time management.
- Provide individual and family counseling for each student/member.
- Assist students/members in developing and navigating natural supports in the community.
- Maintain all necessary records and documentation with accuracy, efficiency, and clarity.
- Use proactive approaches and crisis intervention skills to address challenging situations.
- Communicate students’/members’ needs, issues, and goals to staff members and other parties.
