About The Position

Our programs are grounded in a person-centered philosophy, meeting each individual where they are and supporting them toward greater independence at every stage of life. The Role Vista is seeking a Program Counselor to coordinate, monitor, and support counseling and case management services for members in our Engage Program. This role partners with members, families, and interdisciplinary teams to promote personal growth, independence, community involvement, and achievement of individualized goals. The Engage Program Counselor serves as a case manager, counselor, and advocate, providing individualized support and guidance while ensuring members receive the services and resources needed to succeed.

Requirements

  • Bachelor's degree plus one year of related experience.
  • Valid driver's license and acceptable driving record as defined by Vista
  • CPR and First Aid, PMT Certification or the ability to obtain within 90 days of hire.
  • Strong communication, organizational, and time-management skills.

Responsibilities

  • Develop individualized program plans and goals for assigned members, evaluating progress and identifying programmatic service needs.
  • Implement strategies and supports that promote growth in social/emotional development, vocational skills, life skills, and community involvement.
  • Facilitate direct instruction in areas such as social skills, cognitive development, medication management, money management, and time management.
  • Collaborate with families, team members, and other Vista departments to support member success and coordinate services.
  • Maintain accurate, timely, and comprehensive records and documentation.
  • Utilize proactive problem-solving and crisis intervention skills when needed.
