About The Position

Fraser is seeking a Program Coordinator to join our Community Living team! In this leadership role, you will support individuals served while providing guidance, supervision, and mentorship to staff to ensure high-quality services. You'll help create a positive team culture, support individualized treatment and service planning, and ensure compliance with 245D regulations. Community Living includes both Supervised Living homes and Supportive Living apartments. This role will support one of Fraser’s five Supportive Living apartment settings, where residents live more independently and do not require 24/7 supervision. The Program Coordinator will oversee one apartment complex.

Requirements

  • At least 2 years of professional experience working with individuals with mental health needs.
  • 1 year of supervisory or program coordination experience preferred.
  • Valid driver's license.
  • Must meet 245D qualification requirements, including one of the following: High school diploma/equivalent + 4 years of full-time direct care experience with individuals with disabilities; or Associate degree in Human Services or related field + 2 years of full-time direct care experience; or Bachelor’s degree in human services or related field + 1 year of full-time direct care experience.

Responsibilities

  • Provide leadership, supervision, coaching, and support to Community Living staff.
  • Ensure individuals receive quality services in accordance with their plans, Fraser policies, and 245D regulations.
  • Coordinate intakes, discharges, and services with families, healthcare providers, and other members of the individual's support team.
  • Develop and provide staff training and support professional growth.
  • Support staff with problem-solving, conflict resolution, and day-to-day questions.
  • Ensure required documentation, billing, payroll, and records are completed accurately and on time.
  • Maintain compliance with HIPAA, 245D, and Fraser policies and procedures.
  • Respond to urgent needs and participate in the Community Living emergency on-call rotation.
