About The Position

The Program Coordinator – Case Management reports to the Manager of Case Management and works under general supervision. The role supports RN Case Managers and Social Workers as they collaborate with the interdisciplinary healthcare team across the continuum of care. The coordinator manages payer communication, helps implement transition plans, delivers patient documentation, and performs other duties that support case management departmental functions.

Requirements

• A bachelor's degree and one year of work experience; or a high school diploma or equivalent and 2 to 4 years of work experience.

Responsibilities

• Support RN Case Managers and Social Workers in their work with the interdisciplinary healthcare team across the continuum.
• Coordinate payer communication.
• Support implementation of transition plans.
• Deliver patient documentation.
• Perform other duties in support of case management departmental functions.
