Provides in-hospital education about mental health recovery and resources available after traumatic injury. Conducts in-person or telephone screenings one-month post-discharge for common mental health conditions. Identifies trauma-related symptoms among child and adult survivors of serious traumatic injury using reliable and valid interview-oriented measures. Tracks and interprets patient mental health recovery following discharge using reliable and valid approaches to determine needs for services and progress in Telehealth treatment. Provides referrals for patients who experience other mental health conditions that are not addressed by Telehealth services to other established mental health services through TRRP. Documents the delivery of assessment and referral services and patient responses to services appropriately in each patient's electronic medical record. Maintains evaluation data in centralized database. Enters data directly into database concerning number of patients seen on the floor, followed up at 30 days, and received referrals and mental health screening total scores. Enters clinical notes into electronic medical records system with each patient contact. Attends weekly group supervision meetings via videoconferencing with clinical psychologist for clinical supervision guidance regarding patient interactions, resolving referral issues, and with any crisis situations. Gives presentations on patient interactions.
