Requirements

Bachelor’s Degree from an accredited college or university in Psychology, Social Work, Rehabilitation, or related health/human services field.
One year experience in program development and implementation for DD persons preferred
Must be able to lift, bend, push and pull up to 50 pounds
Valid Ohio Driver’s License – Ohio Bureau of Motor Vehicles.

Responsibilities

Play a meaningful role in the development and implementation of Individual Habilitation Plans (IHP) for all assigned residents
Assess residents’ needs and prepares formal assessment summaries for presentation to the interdisciplinary team
Schedule and chair annual, quarterly and special interdisciplinary team meetings
Ensure the attendance of all required team members
Promptly write and disseminate minutes of all interdisciplinary team meetings
Provide direction and coordination to all professional and residential staff in the development and implement of the IHP and training programs
Write programs and in-service all appropriate staff on implementation of training programs
