About The Position

As a Program Coordinator – Long-Term Care Functional Screener, you'll play a critical leadership role in supporting Wisconsin's Long-Term Care Functional Screen (LTCFS) program. You'll help ensure screeners across the state receive the training, mentoring, and quality oversight needed to deliver accurate, person-centered assessments while maintaining compliance with Department of Health Services requirements. This is more than a screening role—it's an opportunity to influence statewide quality standards, coach and develop professionals, collaborate with healthcare and community partners, and help individuals access the supports they need to thrive.

Requirements

  • Bachelor's degree in Social Work, Human Services, Psychology, Counseling, Nursing, Special Education, or a related field
  • Current Long-Term Care Functional Screen Certification (or ability to maintain certification)
  • Experience completing LTCFS assessments with older adults and/or individuals with disabilities
  • Strong coaching, communication, and organizational skills
  • Passion for empowering individuals to live independently and make informed choices about their lives

Responsibilities

  • Lead and coordinate LTCFS quality, training, and mentoring initiatives
  • Serve as a subject matter expert and resource for screeners across the organization
  • Partner with DHS, ICAs, MCOs, and other stakeholders to maintain screening accuracy and consistency
  • Review cross-validations, rescreens, and quality assurance processes
  • Identify trends, training opportunities, and continuous improvement strategies
  • Support a culture of excellence, compliance, and person-centered service delivery
