About The Position

The Program Coordinator 3 provides direct support and coordination for reentry services within the Community Action Reentry Services Program. This role supports the Housing 360 Project by connecting recently incarcerated individuals to housing and essential community resources that promote successful reintegration. The Program Coordinator works collaboratively with participants, partner agencies, and service providers to reduce barriers and improve access to coordinated, client-centered services. The position focuses on building a unified reentry system where individuals can access housing, employment, and supportive services upon release. Responsibilities include service coordination, referrals, data tracking, and contributing to continuous program improvement efforts. This role requires strong organizational skills and a commitment to trauma-informed, equitable service delivery for justice-impacted individuals. The position is renewable annually, contingent upon continued project funding.

Requirements

  • High School Diploma or GED
  • Two years of case management, social, houseless, or re-entry experience
  • Proficiencies in computers, Microsoft Office products, database software and web tools.
  • Demonstrates tact, diplomacy and empathy when communicating with clients, staff and program partners.
  • Possesses excellent planning, organization and time management skills.
  • Valid driver license, insured vehicle available for use on the job, and a satisfactory drive record for the previous three years are required.
  • Candidate must pass a comprehensive MWVCAA background screening prior to employment.
  • Candidate must pass pre-employment and random drug screenings.

Nice To Haves

  • Preferred experience/knowledge of community partner relationship building, Working with the DOC, Probation or parole officers.
  • Equivalent combination of education and experience will be considered.
  • Preferences given to applicants who have completed Certified Peer Support Specialist training.
  • Basic Life Support/First Aid Certification is required within first 30 days of hire

Responsibilities

  • Coordinates the daily operations of a reentry housing and support program serving recently incarcerated individuals, ensuring a safe, structured, and supportive living environment.
  • Trains, mentors, and supports staff, volunteers, and interns, reinforcing program expectations, policies, procedures, and reentry-focused best practices.
  • Communicates regularly with the Program Manager regarding program operations, facility concerns, client engagement, compliance requirements, and emerging needs.
  • Maintains accurate, timely, and confidential documentation, including client files, case notes, compliance documentation, daily activity logs, mileage logs, and required reports.
  • Utilizes crisis intervention, de-escalation, and trauma-informed approaches when supporting clients experiencing stressors related to reentry, housing instability, or system involvement.
  • Identifies and coordinates referrals for program participants through collaboration with the Department of Corrections, Parole and Probation officers, and other justice-system partners.
  • Assists clients with program intake, housing applications, and documentation necessary to access housing, benefits, treatment, employment, and other reentry resources.
  • Enters and maintains complete and accurate client data, service provision, and housing outcomes in required data systems in compliance with reporting and monitoring requirements.
  • Builds and maintains strong, collaborative working relationships with DOC staff, Parole and Probation officers, housing providers, treatment agencies, employers, and other community partners.
  • Develops professional, respectful relationships with clients that emphasize accountability, engagement, goal progress, and long-term housing and reentry success.
  • Provides program coverage and operational support for program staff as assigned by the Program Manager.
  • Participates in required trainings, meetings, and professional development related to reentry services, housing stabilization, documentation standards, and justice-involved populations.
  • Performs other duties as assigned to support program operations, housing stability, and successful reentry outcomes.
  • Consistent punctuality and reliable attendance are essential requirements for this role.
  • Ability to work effectively and maintain positive, professional relationships with team members and clients.
  • Exercises judgment in determining priority of day-to-day job tasks.
