About The Position

The position of Program Coordinator- Community Services will work with the Program Manager to facilitate services for In-Home and Community Supports, Companion, and Community Participation Supports (CPS). The Program Coordinator will be involved in onboarding new individuals and new direct care staff as well as administrative responsibilities. This role requires flexibility as time will be split between direct care service provision/home visits and Welcome Center based on program needs. Some evenings and weekends may be required.

Requirements

  • 2 years full-time DSP experience
  • Excellent communication skills (oral and written)
  • Working knowledge of the ISP process, incident management, and HCSIS
  • Ability to use all agency devices and technologies
  • Good organizational skills with ability to prioritize tasks
  • Willing to work with all individuals at all sites as needed
  • Driver’s License
  • Act 34 clearance
  • FBI clearance
  • Physical w/Mantoux prior to date of hire
  • Ability to work as a team member
  • Agree to supervision by Program Manager
  • Willingness to carry out the mission and support the values of the Emmaus Community

Responsibilities

  • Onboard new individuals which includes providing direct care until a DSP is identified.
  • Communicate with all ISP team members as needed including family members, Supports Coordinators, behavior specialists, job coaches, day programs, etc.
  • Maintain respectful and meaningful relationships with the individuals.
  • Adhere to and assist the individual with following all health and dietary needs.
  • Plan and provide individuals with opportunities for socialization and recreation both at home and out in the community.
  • Assist individuals with money management, and follow strict policies on protecting and safeguarding client and agency funds.
  • Serve as back up DSP when there are call offs.
  • Complete compliant service notes and goal documentation.
  • Fulfill the 24 hours of training requirement per year.
  • Participate in the development of the ISP, ISP Annual Update & all ISP revisions.
  • Review ISP, ISP Annual Update and all ISP revisions & attend all applicable meetings.
  • Prepare for and attend SIS & IM4Q meetings.
  • Act as Plan Lead for all individuals receiving services through an SCO.
  • Report content discrepancies to the SC and plan team members.
  • Ensure that ISP is implemented as written.
  • Monitor and evaluate services.
  • Complete the quarterly documentation of individuals’ participation and progress toward outcomes.
  • Report changes related to individuals’ needs to the SC and plan team members.
  • Review the ISP & all 90 Day Reviews with the individual.
  • Provide documentation of the plan review to the SC and plan team members.
  • Inform plan team members of the option to decline the ISP review documentation.
  • Recommend revisions that may involve the deletion, addition or modification of an outcome or service.
  • Coordinate the provision of services to meet the needs of the individual.
  • Assist in staff training on ISPs, SEE plans, Restrictive Procedures, Behavior Plans, Incident Management, Crisis Intervention and other topics related to programming.
  • Oversee newly hired DSP shadowing schedules, coordinating with HR; facilitate smooth transition of DSP scheduling.
  • Assist in identifying affordable and creative activities in the community for individuals and communicate with DSPs.
  • Review completion of compliant service notes.
  • Carry out incident management responsibilities including entering reports into EIM, data entry corrective action plans, remediation, etc.
  • Serve as back up Program Specialist for Residential Program Specialist and back up RA as needed.
  • Perform "On-call" responsibilities.
