About The Position

The School Based-Health Center Department partners with three school districts to provide comprehensive health services across seven SBHCs to reduce health inequities by focusing on health prevention and youth empowerment. The Program Coordinator I (PCI) will be a valuable member of the NAHC SBHC team by providing day to day coordination of health center clinical operations and administrative support for the school- based site. NAHC is seeking a bright, motivated, detail-oriented individual to work closely with the Program Manager, School Administration and the SBHC dept. to work as a team of health providers who work to improve the health status of students at NAHC’s school-based clinics. The Native American Health Center is an accredited institution and adheres to the standards of excellence set forth by the Accreditation Association of Ambulatory Health Care (AAAHC) and the Commission of Dental Accreditation (CODA).

Requirements

  • Bachelor’s degree in public health, social work, health education, biological sciences or a related human service field OR equivalent work experience in a community-based program.
  • Excellent organizational skills and attention to detail.
  • Excellent social, verbal, writing and interpersonal skills.
  • Experience with patient care in a health care setting and operational administration.
  • Ability to work independently and as part of a multi-disciplinary team.
  • Able to work effectively with multicultural population of youth, adults, school district personnel and staff to coordinate health center services.
  • Vaccination Level 1: Must be able to provide TB, Physical, Seasonal Flu, proof of vaccination immunity for Measles, Mumps, Rubella, Varicella, TDAP & Hepatitis B prior to start date. If vaccination records are more than 10 years old, titers are required to confirm immunity.
  • CPR certification must be obtained within 90 days and kept current at all times.
  • Able to furnish Vaccination history prior to start date dependent on role.
  • Experience with Windows 2000, Microsoft Office, Excel and Word.
  • Fluency in both English and Spanish, both written and verbal required per operational need.

Responsibilities

  • Greet all members in a courteous and professional manner to create and maintain a welcoming atmosphere.
  • Schedule member appointments appropriately, answer and return telephone calls in a timely manner, and inform members of processes and procedures.
  • Register members into electronic health records EPIC in a manner that ensures accuracy and thoroughness and updates member registration information annually or as information changes. Inform members of Notice of Privacy Practices and obtain the members signed acknowledgement statement.
  • Screen members’ eligibility for possible care coverage prior to scheduling appointments (e.g., county or state programs; sliding scale) and facilitate enrollment into appropriate publicly-funded programs and follow enrollment and program guidelines.
  • Check-in members for their scheduled appointments in a timely manner and complete all flows for the member appointment such as; verifying member demographics, processing payments, insurance verifications, payer code selection and printing encounter labels.
  • Manage medical, dental and behavioral health schedules in departmental Electronic Practice Management (EPM) system; OCHIN EPIC and Dentrix.
  • Responsible for coordinating the day to day functioning of the SBHC, which includes organizing patient documents, scheduling appointments and provide administrative support for SBHC services.
  • Actively participate in morning huddles and provide appropriate patient case management information that will assist with patient care delivery.
  • Work collaboratively with both Program Manager and clinical providers (medical, dental, behavioral health) to support with patient follow up and case management.
  • Engage regularly with the SBHC Program Manager(s) to ensure services are aligned with overall growth and forward direction of the agency and in compliance with department, agency, and funding agency regulations.
  • Work collaboratively with Program Manager to ensure any required data by our funders is and evaluation teams is occurring and submitted in a timely manner.
  • Work closely with site Program Manager and school site staff to organize, coordinate, and promote outreach activities and health promotion events with school administration and partner agencies.
  • Refer students and their families to appropriate community agencies; acts as liaison with such organizations and advocates for members and/or their families.
  • Provide triage and first aid when appropriate for students.
  • Support program site as needed with miscellaneous administrative duties such as data entry, tracking information, and scheduling activities.
  • Actively participate in staff meetings, workshops, trainings; as well as coordinate activities with collaborating agencies.
  • Demonstrate understanding and apply working knowledge of safety policies and ensuring safe member practices.
  • Safely performs all duties; follows required protective protocols to ensure personal safety as well the safety of others.
  • Must maintain compliance with ergonomic safety standards; be mindful of posture and regularly practice ergonomic stretches.
  • Actively participate in internal quality improvement teams and work with members proactively to drive quality improvement initiatives in accordance with the mission and strategic goals of the organization, federal and state laws and regulations, and accreditation standards, when assigned.
  • Keep all protected health information (PHI) confidential and abide by HIPAA policies for the release and disclosure of any PHI. Will report unauthorized use of disclosure of PHI immediately, to supervisor or HIPAA security officer.

  • At all times demonstrate cooperative behavior with supervisors, subordinates, colleagues, clients and the community.
