About The Position

The Community Support Services Program Coordinator oversees and supports community support employees providing community-based behavioral health rehabilitation and support services to adults, youth, and families with behavioral health needs, including individuals with severe and persistent mental illness and children and families requiring rehabilitative and family-focused services. This position coordinates daily program operations, staff support, service delivery oversight, documentation compliance, training, and workflow development to ensure services are responsive to member needs and compliant with Nebraska Medicaid, Managed Care Organization (MCO), agency, and accreditation standards. The Community Support Services Program Coordinator collaborates closely with licensed behavioral health professionals and serves as a liaison among community support employees, behavioral health providers, MCOs, hospitals, schools, families, and community partners to promote continuity of care, recovery, stability, community integration, and member engagement.

Requirements

  • Bachelor’s degree in Social Work, Psychology, Human Services, Criminal Justice, Behavioral Health, or a related field; or an equivalent combination of education and relevant experience as permitted by applicable regulations and agency standards.
  • Experience working with adults with severe and persistent mental illness or individuals with complex behavioral health needs.

Nice To Haves

  • Master’s degree in a behavioral health-related field.
  • Previous supervisory, leadership, or program coordination experience.
  • Experience with Nebraska Medicaid behavioral health rehabilitation services or Community Support Services.
  • Familiarity with crisis intervention, recovery-oriented care, trauma-informed care, and care coordination models.

Responsibilities

  • Coordinate daily operations and workflow of community support employees providing community-based behavioral health rehabilitation and support services.
  • Assign and monitor caseloads, service utilization, and staff productivity to support service intensity, continuity of care, member engagement, authorization compliance, and quality service delivery.
  • Provide ongoing coaching, mentoring, guidance, onboarding, and training to staff regarding service delivery expectations, documentation standards and compliance, professional boundaries and safety practices, crisis intervention and engagement strategies, care coordination and community integration, recovery-oriented, person-centered, trauma-informed, and family-focused practices, and Nebraska Medicaid Community Support service requirements.
  • Monitor service delivery and documentation to ensure alignment with assessed member needs, treatment goals, authorization requirements, Medicaid standards, MCO expectations, agency policies, and accreditation requirements.
  • Support staff in addressing barriers related to mental health stability and crisis prevention, medication and appointment adherence, family and natural support systems, transportation and access to resources, independent living skills and community integration, member engagement and re-engagement efforts, and service coordination and linkage to community supports.
  • Facilitate communication and coordination among community support employees, licensed behavioral health professionals, families, schools, hospitals, MCOs, external providers, and community resources to promote continuity of care and effective treatment planning.
  • Participate in staffing meetings, care coordination meetings, treatment planning, utilization reviews, audits, retrospective reviews, billing compliance activities, and quality improvement initiatives.
  • Collaborate with clinical leadership regarding service appropriateness, documentation concerns, member eligibility, safety concerns, workflow development, policy updates, and program improvement opportunities.
  • Support program development through identification of service trends, workflow needs, staff education opportunities, and service quality enhancement initiatives.
  • Promote positive relationships between the agency and community partners to support collaboration, awareness, referrals, and engagement opportunities.
  • Promote person-centered, recovery-oriented, and trauma-informed approaches to service delivery.
  • Create, maintain, and share as appropriate a dynamic self-care plan.
