About The Position

Program Clinical Coordinator (Case Management) Program Name: California MENTOR Family Home Agency Office location: Alhambra Program area: East LA, Whittier, Pico Rivera, Montebello (must be willing and able to travel for field visits in these areas regularly) Pay: $45,800 annual salary (overtime eligible) Everybody needs a job but only extraordinary people work here. Requirements

• Bachelor’s degree in human services or related field.
• One year of work-related experience working with individuals with intellectual and developmental disabilities
• High School diploma/GED
• At least two years of experience in a similar program serving individuals with developmental disabilities
• Valid California driver's license in good standing
• Reliable vehicle with proof of valid registration and insurance
• A reliable, responsible attitude and a compassionate approach and a commitment to quality in everything you do
• On-call availability required after hours and on weekends

• Experience working directly with individuals with intellectual or developmental disabilities

Responsibilities

• Coordinate the development, review, and implementation of services, case management, behavior intervention plans, and treatments provided to individuals served.
• Oversee all services provided to individuals and their families, monitor quality of care delivered, ensure compliance with contractual terms, and monitor regulatory program compliance.
• Assist in the development of quality services and engaging activities that meet the individual served needs.
• Build and maintain relationships with families and external case managers.
