About The Position

At Northeast Arc, our mission is to ensure that individuals with disabilities have every opportunity to live, work, and thrive in their communities. As a Program Case Manager/Driver on the Journeys team, you will play a meaningful, hands-on role in making that mission a reality each day. In this position, you will provide direct supervision and skill-building support to participants in Northeast Arc's community-based day and work supports program, while also taking responsibility for safely transporting participants to and from program sites and community locations. This is a role for someone who brings both a caring presence and a reliable sense of responsibility to their work, someone who understands that the journey matters just as much as the destination.

Requirements

  • High School Diploma or equivalent required
  • A minimum of 1 year of experience working with individuals with intellectual and/or developmental disabilities, or other related experience preferred
  • Excellent organizational and interpersonal skills; effective communicator in both person and in writing
  • Team player who is flexible and able to adapt to a changing schedule
  • Ability to communicate in English and/or ASL via direct communication
  • Working knowledge of Microsoft Office products and associated applications
  • Valid driver's license and reliable transportation required
  • Must be able to stand and/or walk up to one hour uninterrupted
  • Must successfully complete and pass all required background checks as a condition of employment.

Responsibilities

  • Provide direct supervision and individualized support to participants in Northeast Arc's community-based day and work supports program
  • Complete daily transportation runs for assigned participants in compliance with HST/MART/DDS regulations
  • Facilitate skill development activities and meaningful opportunities tailored to participants' individual goals
  • Support participants at volunteer sites, work sites, and community outings
  • Maintain accurate program records and daily documentation
  • Communicate effectively and respectfully with participants, their families, and program staff
  • Operate program vehicles safely and in accordance with Northeast Arc and regulatory requirements
  • Collaborate with the Program Director to ensure consistent, high-quality service delivery
