About The Position

Yee Hong Centre for Geriatric Care (Yee Hong) is a leader in senior care, providing exceptional, culturally inclusive services. They offer long-term care, community programs, and aging-in-place initiatives, focusing on compassionate, person-centered care for diverse communities. Yee Hong has been a provider of culturally appropriate senior care since 1994, operating long-term care beds, seniors' apartment units, campuses of managed care, a hospice residence, and various community-based senior and caregiver support services across the Greater Toronto Area, serving over 15,000 individuals annually. The Program Assistant (Casual) role, under the direction of the Program Manager/Program Coordinator, is responsible for planning and participating in activities and providing high-quality services to clients of the Day Program.

Requirements

  • Diploma in Social Work or Social Services, health sciences, activation, or equivalent.
  • Experience in geriatric rehabilitation is essential.
  • Excellent customer service skills.
  • Ability to work independently and contribute effectively as a team player.
  • Computer literacy in Microsoft Office Suite.
  • Recent certification in C.P.R.
  • Recent satisfactory Police Vulnerable Sector Screen result.
  • Fluency in English and ability to converse in Chinese.
  • Must be able to work across different Yee Hong sites as needed.

Responsibilities

  • Participates in the planning of appropriate activities for clients of the program.
  • Implements and ensures smooth operation of the activities in a safe, hygienic and supportive environment.
  • Provides timely and appropriate crisis intervention or behaviour management and ensures that the clients' physical, social and emotional needs are appropriately met.
  • Provides timely and appropriate support/information and referral/placement/outreach to clients/volunteers (as appropriate) to ensure that the clients/volunteers' social/ recreational/development needs are appropriately met.
  • Works closely with client and caregiver in ensuring appropriate design of care plan/program plan and ongoing monitoring of plan.
  • Ensures appropriate assessment, placement, matching and support for volunteers.
  • Provides effective guidance, training and support to the volunteers in the program.
  • Maintains up to date and accurate files, records and statistics on the program.
  • Maintains accurate record of fees and expenditures.
  • Complies with risk management policy for health and safety and infection control.
  • Keeps up-to-date with developments and best practices in the field.
  • Other duties as assigned.
