Requirements
- One (1) year of Home Health Aide experience in home, community based or LTC setting
- Compassionate, kind and patient focused
- Exceptional customer service and interpersonal skills
Responsibilities
- Assist in planning and implementing outings for Participants
- Provide daily documentation of patient care
- Perform patient care such as feeding, grooming and transferring as determined by their care plan
- Prepare and distribute meals and snacks
- Assist with recreation/leisure activities
- Report any changes in patient status to Senior Program Assistant or Activity Coordinator in a timely manner
- Greet participants upon arrival
- Remove coats and guide to program area
- Prepare participants for departure
