About The Position

The Program Nurse - LPN will coordinate the delivery of nursing and social services for adult day participants. This is a part-time position, 22.5 hours per week, working 3 days per week.

Requirements

• Graduate of accredited nursing school
• 2-3 years of nursing experience
• State of Connecticut Nursing License
• CPR certification
• Ability to juggle multiple tasks and accomplish deadlines in an organized fashion.
• Utilizes Microsoft suite, including outlook, and excel; flexibility and initiative to adapt to changing software technology.
• Ability to travel to client homes and other community sites as necessary related to networking and intakes.
• Availability of personal vehicle for work related travel (mileage reimbursed). Travels primarily within Fairfield and New Haven counties.

Nice To Haves

• Experience working with the elderly and those with dementia/Alzheimer's Disease
• Work in a community-based setting
• Knowledge of services and entitlements for frail elderly
• Bilingual Spanish

Responsibilities

• Conduct new client intakes and assessments to determine the appropriateness of referrals and encourage participation at start of program and once enrolled.
• Conduct/assess the nutritional, medical, physical and mental functional needs of the clients during the initial assessment and ongoing as necessary within scope of nursing practice.
• All other related duties as assigned.
