Depending on the visit, Provider direction, and whether another assistant is present, the RN may perform scribe duties, circulator duties, or both. Scribe duties include documenting the Provider’s patient encounter, ensuring the Provider reviews and signs the chart, monitoring and returning patient calls, monitoring and responding to EMR messages, and performing related duties. Scribe documentation does not include prescriptions. Circulator duties include updating the patient’s Face Sheet Plan after visits; documenting warnings, violations, or inappropriate behavior; updating medical history ICD codes; reconciling medications; performing pill counts; entering PQRS codes; recording vital signs; completing referral letters; sending prior-authorization messages for back braces; scheduling follow-up appointments; monitoring and responding to patient calls and EMR messages; and performing related duties. During procedures, the RN administers moderate sedation. Responsibilities include applying knowledge of anatomy, physiology, pharmacology, cardiac arrhythmia recognition, and sedation-related complications; assessing patient care needs before, during, and after sedation; using oxygen-delivery devices; recognizing potential complications; and assessing, diagnosing, and intervening in complications or undesired outcomes in accordance with orders and institutional protocols.
