San Jose Behavioral Health in San Jose, CA is looking for a PRN House Supervisor (RN) to join their nursing team. Requirements

  • Current registered nursing license in State of facility required
  • 1 year of experience as an RN in any setting required
  • Current certification or license to practice within the state where the facility is located.
  • CPR and de-escalation/restraint certification required (training available upon hire and offered by facility).
  • First aid may be required based on state or facility.

Nice To Haves

  • BSN from accredited school of nursing preferred.
  • Greater than 1 year of experience as an RN strongly preferred.
  • 1 year in psychiatric/recovery programs preferred.
  • Some experience as a charge RN or other leadership position strongly preferred.

Responsibilities

  • Responsible for day-to-day oversight of nursing operations of the facility and ensures staffing is adequate to provide care.
  • Oversees the delivery of behavioral health/nursing services in a positive, empathetic, and professional attitude toward customers at all times.
  • Collaborates with interdisciplinary treatment teams, other departments, and administration to ensure that all residents’ physical, biopsychosocial, age, developmental, and cultural needs are met and works to resolve customer complaints.
  • Provides leadership to assure compliance with local, state, and federal regulations, as well as nursing practice standards.
  • Recognizes that patient safety is a top priority.
  • Anticipates and effectively manages changes in census and acuity and allocates nursing resources based on measurement of patient acuity/care needed.
  • Role models expectations related to customer service and demonstrates a sense of urgency related to the importance of patient safety.
  • Enforces and interprets policies for the delivery of patient and resident care.
  • Ensures proper staffing for each unit, keeping staff ratio, appropriate staffing for patient care needs, covering call-offs and vacation time.
  • Ensures chart checks are completed, seclusion/restraint log is up to date, nursing assessments are completed, as well as MAR’s and report sheets are kept accurate and up to date.
  • Rounds are completed at the beginning of shift and perform random audits.
  • Assists nursing leaders to make sure all staff are oriented and competent to perform required duties, which may include coaching staff.
  • Provides mentoring and training as needed.
  • Assists the nursing leadership team by ensuring facility compliance with professional, regulatory, and governmental standards.
  • Ensures the effective delivery of competent, compassionate, and safe care by monitoring and evaluating patient care processes and outcomes on the patient care units.
  • Coordinates all care rendered to patients, including physician orders and diagnostic information, and ensures consultations are ordered.
  • Documents medical, behavioral health information, and nursing care.
  • Administers medications and maintains records in accordance with company policies and procedures.
  • Assists in developing a plan of care with specific and measurable goals, objectives, and interventions defining actions unique to each patient's needs.
  • Assists in updating and revising the plan of care as goals/objectives are met or when the patient's condition changes.
  • Prioritizes tasks and delegates staff to ensure quality patient care.
  • Acts as the leader on shift for all admissions and discharges, as well as informing the physician of changes in the patient’s conditions.
  • Adjusts staffing accordingly.
  • Utilizes administrator-on-call for situations that require additional attention.
  • Provides direct care as needed.
  • Performs other functions and tasks as assigned.
