About The Position
Concierge Nursing Referral Services, a specialized branch of TheKey, connects elite physical therapists with discerning, high-net-worth clients seeking exclusive, in-home care. This role offers the opportunity to take control of your schedule, clients, and income by providing high-end private duty care. Unlike traditional settings, this role allows therapists to focus 100% of their energy on delivering high-touch, unhurried, personalized therapy without the constraints of insurance red tape, due to clients paying privately. Therapists can earn high-yield rates, with potential earnings of up to $250+ per evaluation and $200+ per follow-up visit, though rates vary by client and are negotiated prior to each case. The job is a 1099 Independent Contractor position, offering flexibility for per-diem, short visits, or ongoing cases.
Requirements
- Active Physical Therapist (PT) license in the State of California (Required).
- 2+ years of PT experience strongly preferred.
- Proven ability to work autonomously with exceptional clinical judgment.
- High reliability, flawless communication, and a polished, professional demeanor suited for VIP clientele.
- Tech-savvy with basic computer skills for quick, efficient digital charting.
- Proof of eligibility to work in the U.S.

Responsibilities
- Design and implement highly customized treatment plans tailored to your client’s lifestyle and functional goals.
- Provide hands-on, high-quality interventions to maximize patient recovery and independence.
- Coordinate directly with physicians, family members, and multidisciplinary care teams to deliver a cohesive client experience.
- Guide and train clients and their families to maintain long-term safety, wellness, and functional mobility.
- Maintain clear, prompt documentation in accordance with referral standards.
