About The Position

Concierge Nursing Referral Services, a specialized branch of TheKey, is seeking an experienced LPN for a Care Management case in Fairfax County. This role offers an opportunity to escape the burnout of traditional hospital settings and provide high-end, personalized, one-on-one care to discerning VIP clients in their homes. As an independent contractor, you will have complete control over your schedule, income, and case selection, focusing on delivering premier clinical care with true 1:1 patient interaction.

Requirements

• Active LPN License in good standing.
• 2+ years of nursing experience (strongly preferred).
• Active CPR / BLS Certification.
• Proof of eligibility to work in the U.S.
• Ability to work independently with confidence, professionalism, and minimal supervision.
• Strong commitment to punctuality, reliability, and discretion.
• Comfortable with basic computer skills and digital documentation tools.

Nice To Haves

• Portuguese bilingual skills preferred.

Responsibilities

• Provide expert, compassionate 1:1 skilled nursing care, including ADLs, vital monitoring, wound care, medication management, and doctor-prescribed treatments.
• Coordinate client needs, including medical supplies, DME, and transport to appointments.
• Maintain clear, timely documentation and communicate effectively with families, physicians, and the support office.
• Empower patients and their families through personalized health education and training.
