Requirements

  • Possess a current registration with the College of Nurses of Ontario (CNO) as a Registered Nurse
  • A minimum of 3-5 years of supervisory experience or equivalent clinical leadership experience in home and community healthcare.
  • Advanced Clinical Expertise: Demonstrated ability to deliver acute and advanced care in the home or community setting.
  • Strong Leadership Skills: Proven track record in team development, supervision, and fostering collaboration.
  • Exceptional Organizational Skills: Capacity to manage time-sensitive visits, prioritize patient needs, and maintain comprehensive records.
  • Commitment to Quality Care: Passion for delivering high-quality, safe, and patient-centered services.
  • Strong proficiency with Microsoft Office Suite and experience with scheduling/documentation systems.
  • A valid Alberta driver's license and access to a reliable, insured vehicle.
  • Clinical Expertise: Strong assessment, care planning, and medication reconciliation skills.
  • Problem Solving: Analytical and critical thinking to identify service gaps and implement improvements.
  • Communication: Excellent verbal and written skills for interacting with diverse stakeholders.
  • Organizational: Superior time management and attention to detail for managing competing priorities.
  • Team Collaboration: Commitment to fostering interdisciplinary teamwork and patient-centered care.

Responsibilities

  • Provides expert clinical leadership and delivers high-quality, client-centered care to individuals with complex needs in the community.
  • Combines clinical excellence, leadership, and mentorship to support sustainable care transitions, hospital avoidance, and optimized patient outcomes.
  • Leverages an advanced nursing skill set to deliver time-sensitive, comprehensive patient visits—often within 24 hours—to stabilize complex patients, conduct thorough assessments, and develop or adjust individualized care plans.
  • Delivers advanced nursing care for complex patients, including urgent, time-sensitive visits and care plan adjustments.
  • Conducts comprehensive, evidence-based assessments and develops individualized care plans in collaboration with clients, families, and interdisciplinary teams.
  • Mentors, supports, and supervises team members to enhance clinical skills, confidence, and professional growth.
  • Leads accurate, timely clinical documentation and ensures compliance with regulatory and organizational standards.
  • Champions innovative, client-centered care models that support hospital avoidance, safe transitions, and continuous quality improvement.
  • Develops, implements, and adjusts individualized care plans in collaboration with patients, families, and interdisciplinary teams.
  • Identifies additional service needs and ensures appropriate resources are mobilized to optimize patient outcomes.
