About The Position

CASES is seeking a Primary Clinician (OnTrackNY), a licensed Master’s or Ph.D.-level clinician, who is passionate about supporting adolescents and young adults in the early stages of serious mental illness. In this role, you will make a meaningful impact by working with individuals within two years of experiencing non-affective psychosis, helping them and their families navigate recovery from the very beginning. As a key member of a small, multidisciplinary team, you’ll provide coordinated, assertive, and person-centered care grounded in shared decision-making. You will serve as the primary clinician for clients and families, leading assessments, providing outreach and engagement, and offering case management, supportive psychotherapy, and cognitive behavioral interventions—all within a highly collaborative, community-based model. We’re looking for clinicians with training and experience working with adolescents or young adults with serious mental illness, a background in community-based mental health, and a strong commitment to recovery-oriented, evidence-based care. Requirements

  • Licensed Masters or Ph.D. in an appropriate field (work, psychology, mental health counseling)
  • Training and experience working with adolescents and/or young adults with serious mental illness.
  • Experience in community-based mental health services.

Responsibilities

  • Conduct telephone and in-person eligibility screening of participants referred to the program.
  • Refer participants ineligible for the program to other appropriate community providers.
  • Serve as the Primary Clinician for a subset of program participants.
  • Serve as the point person for the participant and their family.
  • Develop and modify treatment using a shared decision-making process with participants.
  • Provide services based on participant and family preferences and needs, including supportive individual and family therapy in the office (on-site) or community (off-site).
  • Provide behavioral interventions and take an especially active role in the family support domain.
  • Conduct clinical and needs assessments to determine the participant’s social functioning and substance use level via a shared decision-making (SDM) framework.
  • Engage in safety and wellness management planning with participants.
  • Connect participants and families with the services and support as needed.
  • Document services in appropriate databases and monitor due dates ensuring timely updates and reviews.
  • Attend weekly team meetings.
  • Participate in ongoing training and supervision.
  • Provide Social Skills Training, Substance Abuse Treatment, Behavioral Activation, Coping Skills Training, and Psychoeducation.
  • Work with participants and families for the transition from the team to other services and support that ensures stable links at discharge.
  • Participate in a department rotation schedule, wherein clinicians are expected to periodically remain on alert for client emergencies after hours, ensuring timely response and support when necessary.
  • Help ensure we maintain a safe working environment by reporting any hazards or unsafe conditions, e.g., liquid spill in hallway, to the appropriate facilities staff member or supervisor.
  • Recruit participants for the program through community education and presentations at potential referral sources.
  • Establish and maintain a database of referral sources with up-to-date contact information.
  • Serving as the primary contact person for referral sources.
  • Facilitate groups providing psychoeducation to participants and families.
  • Rotate on-call responsibilities with other licensed team members.
  • Provide psychoeducation regarding first-episode psychosis (FEP) and the goals of the OnTrackNY program.
  • Collaborate with collateral services as needed.
