About The Position

Reporting to the Health Services Lead of Primary Health Care, the Primary Health Care (PHC) Coordinator will provide support to advance the strategic priorities in the Primary Health Care, Chronic Disease Management and Wellness and Department of Family Practice portfolio. The PHC Coordinator will have responsibility for coordination of the planning, ongoing development and implementation of programs, services and initiatives. The incumbent works closely with Primary Health Care leadership, providers, teams and stakeholders to ensure localized support for planning, coordination, facilitation, and evaluation of PHC projects/initiatives. The Coordinator is responsible for developing and maintaining strong working relationships with health care staff, Provincial and other Zonal and NSH departments, external individuals and partner agencies. The incumbent will support the Northern Zone's quality improvement initiatives for collaborative family practice and chronic disease management and wellness teams in Primary Health Care. The Primary Health Care Coordinator will work collaboratively with family physicians and other health care providers and teams to support change management strategies that are inherent in the services plan for primary health care. Please note this position requires travel to facilities within Colchester County. A valid driver’s license and access to reliable transportation is required.

Requirements

  • Bachelor's degree preferably from a health discipline/health related science field required or an equivalent degree and experience may be considered
  • Two years' of recent related experience required
  • Organizational and project management skills.
  • Superior time management skills in order to prioritize and complete assignments
  • Broad knowledge of the health system with high level understanding of population health approaches, community development, primary health care, health prevention and promotion, and disease management
  • Sound knowledge and skill working in an interdisciplinary setting
  • Proficiency in Microsoft Office (Word, Access, PowerPoint, Excel).
  • Demonstrated high degree of independent decision making, self-direction, motivation and team building/leadership skills
  • Understanding of Behavior Change Techniques
  • Valid Nova Scotia Driver’s License

Nice To Haves

  • Chronic Disease Management experience preferred
  • Experience in Practice Facilitation including coaching, facilitation, change management strategies, and quality improvement is an asset
  • Experience working in a family practice setting with family physicians an asset
  • Experience with panel management an asset
  • Desktop publishing an asset
  • Proficiency in PHC digital tools (EMR-Med Access and Accuro, Pomelo, Ocean portal and Smartsheet) an asset
  • Competencies in other languages an asset, French preferred.
