About The Position

Hartford HealthCare at Home is seeking a Registered Nurse to provide personalized homecare to patients in Union, Ashford, Eastford, Chaplin, Hampton, and surrounding towns. Primary Care Nurses use their education and training to deliver care in an autonomous, rewarding environment. The organization’s Person-Centered Care Model supports integrated care and professional growth, with technology used to provide quality care in clients’ homes.

The role requires a valid Connecticut RN license, at least one year of RN experience, a valid driver’s license, reliable transportation, and computer proficiency. Homecare experience is preferred. Graduate RNs with less than one year of experience are automatically enrolled in the Nurse Residency Program.

Responsibilities

• Perform comprehensive assessments to establish an individualized care plan.
