About The Position

Hartford HealthCare at Home is seeking a Primary Care Nurse to provide skilled nursing care to clients in home and community-based settings. Following a person-centered care model, the nurse ensures the delivery of outcome-based, high-quality, cost-effective care and collaborates with team members to achieve exceptional outcomes. The role uses the agency’s standards for client-record documentation and carries out established plans of care, including assessing disease progress, evaluating responses to treatment, teaching patients and caregivers, and coordinating services with providers and community resources.

Requirements

Graduate of an Associate’s Degree or diploma program in nursing.
One year of nursing experience or one year of nursing experience in a specialty.
Registered nurse with a license to practice in the state of Connecticut.

Nice To Haves

Graduate of an NLN-approved BSN program.
Medical/surgical or home care experience.

Responsibilities

Provide skilled nursing care to clients in home and community-based settings.
Maintain client records in accordance with the agency’s documentation standards.
Make follow-up visits to provide skilled care, assess disease progress, and evaluate responses to treatment.
Support patients and families through teaching to promote and restore health and prevent disease and disability.
Carry out the plan of care and teach patients, reinforcing client and caregiver education.
Refer, collaborate, and coordinate client care services.
Consult with physicians regarding client needs and plans of care.
Communicate and conference with the Nurse Care Coordinator and others involved in patient care.
Direct LPNs in carrying out the established plan of care.
Direct Home Care Aides and supervise the plan of care.
Refer clients and families to other community resources as needed.
Attend and participate in in-service education programs.
