About The Position

Central City Concern is seeking compassionate, mission-driven Medical Providers for the 16th & Burnside Recovery Center, a residential trauma-informed recovery facility in Portland. As a Provider, you will collaborate with the Medical Director to offer high-quality, person-centered care to individuals facing challenges with substance use, mental health, and homelessness. This is a locum tenens opportunity.

Requirements

  • Active Oregon licensure (or eligibility) and DEA certification.
  • Commitment to health equity, harm reduction, and collaboration.
  • Current DEA certification.
  • Must have current BLS certification prior to start.
  • Must meet CCC privileging requirements as required by FTCA.
  • Available to work during standard hours.
  • Must pass a pre-employment drug screen, TB test, and background check. This includes clearance by the DHS Background Check Unit.
  • The OAR prohibits A&D program staff from having within the previous 3 years a DUI conviction or diversion, a felony conviction, or a conviction under the Uniform Controlled Substances Act.
  • Must generally have the ability to walk, bend, stoop, kneel, squat, twist, reach, pull and lift heavy objects, and climb stairs with reasonable or no accommodation.
  • Must adhere to agency non-discrimination policies.
  • Ability to effectively interact with co-workers and clients with diverse ethnic backgrounds, religious views, political affiliation, cultural backgrounds, lifestyles and sexual orientation and treat individuals with respect and dignity.
  • Graduate of an accredited Nurse Practitioner Program or Physician Assistant program required of all Nurse Practitioner applicants.
  • If candidate is a Nurse Practitioner, graduate of an accredited school of nursing and has unrestricted licensure to practice medicine in Oregon or eligibility for such licensure.
  • Current Advanced Practice Registered Nurse (APRN) Licensure with National certification or licensed to practice as a Physician’s Assistant in Oregon, with at least 2 years’ experience.
  • Board eligibility or certification in Internal Medicine or Family Practice.
  • Graduation from a CNME-accredited, four-year Naturopathic Medical program.
  • Must have completed a CNME-certified primary care residency program.
  • Licensed naturopathic physician in the State of Oregon.
  • Maintain an Active DEA Certification.

Nice To Haves

  • Strong foundation in addiction medicine and mental health care.
  • Experience with underserved and homeless populations.
  • A strong background in mental health and addiction medicine is preferred, as well as experience working with homeless and underserved populations.
  • Demonstrated skills in primary care medicine with a minimum of 2 years’ experience preferred.
  • Experience in urgent care and/or primary care practice.
  • Knowledge of de-escalation methods or willing to be trained in these methods.

Responsibilities

  • Perform admission evaluations, including buprenorphine induction.
  • Manage chronic and acute conditions.
  • Partner with a multidisciplinary team.
  • Be available for on-call medical support.
  • Provide admission orders for all new patients, including orders for the administration of Buprenorphine.
  • Determine and formulate health histories and perform physical exams on all newly admitted patients.
  • Order, interpret, and evaluate diagnostic tests to identify and assess patients’ clinical problems.
  • Formulate treatment plans based on available clinical data and work cooperatively with patients to implement treatment plans.
  • Conduct on-going health care and clinical management of stable, chronically ill patients.
  • Determine and manage minor and/or acute illnesses and initiate treatment.
  • Conduct initial care for emergency situations such as illness or injury and initiate arrangements for continuing care.
  • Collaborate with an interdisciplinary team with regard to patient treatment.
  • Prescribe medication or other forms of treatment as necessary.
  • Keep appropriate records.
  • Participate in compliance, quality assurance, and quality improvement activities as directed.
  • Take calls from RNs and medical staff even when off-site when working.
  • Adhere to all state and federal privacy regulations, including HIPAA and 42 CFR Part 2, and to CCC policies and agreements regarding confidentiality, privacy, and security.
  • Support compliance with all privacy and security requirements pursuant to community partners’ and outside providers’ patient confidentiality agreements, including privacy and security requirements for EMR access.
  • Immediately report any breach of protected health information or personal identification information of any person receiving CCC services by CCC or an outside provider to the CCC Compliance Department, as well as to your supervisor or their designee.
  • Perform other duties as assigned.
