About The Position

Senior Doc, part of Brigade Health, is hiring compassionate Nurse Practitioners (NPs) and Physician Assistants (PAs) to provide high-quality mobile primary care to older adults in the comfort of their homes, assisted living, and senior communities. If you’re passionate about geriatric care, senior health, chronic disease management, dementia care, and relationship-based medicine, we’d love to connect with you.

Requirements

• Active NP or PA license
• 1+ years of primary care, geriatrics, home health, hospice, SNF, or senior care experience preferred
• Strong communication and clinical skills
• Valid driver’s license & reliable transportation

Responsibilities

• Provide mobile primary care visits to seniors

• Maintain accurate EMR documentation
