About The Position

Yale New Haven Health is inviting internal medicine-trained physicians to join an expanding established primary care practice. This collegial practice consists of 4 physicians and 2 APRNs, with a 1:1 support staff model. The position offers excellent work-life balance with 36 patient hours per week, 4 hours of administrative time, and a four-day work week option. Physicians will see 16-20 patients per day with light, shared phone-call responsibilities. The practice utilizes the EPIC EMR system and an ambient AI scribe service. There are opportunities for leadership and academic involvement as an affiliate of Yale University School of Medicine.

Requirements

  • Degree of MD or DO with ABIM-recognized internal medicine residency (three years minimum) or ABFM-recognized family medicine residency (three years minimum)
  • Valid American Board certification in either internal medicine or family medicine and renewed (if needed) prior to expiration
  • Valid and unrestricted New York medical license
  • BLS certification
  • Valid DEA registration and Controlled Substance Certificate (or in the process of obtaining)

Responsibilities

  • Evaluate patients, and treat acute and chronic illnesses
  • Orders, performs and interprets diagnostic tests
  • Involves patients and their families in care and treatment decisions
  • Creates treatment plans and completes orders in patient's charts
  • Collaborates on improvements in care delivery and service with staff, other physicians and management
  • Initiates appropriate specialty referrals
  • Provides clinical information to the interdisciplinary team in a clear and timely manner
  • Ensures that proper patient documentation is timely, accurate and complete
  • Works with practice management to ensure proper compliance with billing, coding and CPT code assignment
  • Achieves and maintains optimal patient access
  • Provides interdepartmental medical consultation as requested
