About The Position

ChenMed is a physician-led, mission-driven primary care organization focused on delivering affordable, value-based care to seniors. The Primary Care Physician (PCP) provides care for older adults with acute and chronic illnesses and is accountable for the clinical outcomes, cost-effectiveness, and overall care of a patient panel of up to 450 people. The PCP builds trusting relationships, coaches patients toward health goals, and leads care teams to support improved outcomes. The role is measured against patient admissions, appointment capacity, patient experience, clinical gap closures, and medical costs, and contributes to a positive, accountable, and patient-focused center culture. The PCP works independently and collaboratively with other healthcare professionals, managers, medical directors, specialists, hospitalists, and care teams. The physician coordinates care across office, hospital, skilled nursing, long-term care, and homebound settings; supports center operations and coverage; and participates in quality, risk-management, and clinical programs. The position requires compliance with organizational standards, regulatory requirements, and clinical policies. The PCP also supervises or collaborates with an Advanced Practice Practitioner and remains accountable for the APP’s actions. The position requires a medical degree and an active or obtainable state medical license, board eligibility, and a current DEA number. BLS certification is required within the first 90 days. The role involves local travel as applicable, two weeks of national travel for initial training, and occasional regional or national travel for recruiting or training. Evening, weekend, or holiday work may be needed. The advertised annual salary range is $221,141–$315,915; final compensation depends on relevant factors, and the position may also be eligible for bonuses or commissions.
