Requirements

  • Board Certification in Family Medicine or Internal Medicine
  • Significant clinical experience in a primary care setting
  • A passion for patient-centered care
  • Experience with—or enthusiasm for—a value-based care model
  • Strong electronic medical record (EMR) documentation skills
  • Excellent communication and interpersonal abilities
  • A collaborative, team-oriented mindset
  • Flexibility, humility, and a willingness to embrace innovation and continuous improvement

Nice To Haves

  • Effectively identify and document risk-adjustment diagnoses
  • Support patients through preventive care and chronic disease management
  • Help eligible patients enroll in advanced care management programs
  • Progress toward an average of approximately two patient visits per hour while maintaining a high standard of care

Responsibilities

  • Provide comprehensive primary care services for adult patients
  • Deliver patient-centered care with a focus on prevention, chronic disease management, and wellness
  • Accurately document patient encounters and support value-based care initiatives through quality coding and documentation
  • Collaborate with an interdisciplinary team to provide coordinated, high-quality care
  • Participate in population health initiatives and identify opportunities to improve patient outcomes
  • Build long-term relationships with patients while delivering compassionate, evidence-based care
