About The Position
Conviva Primary Care, a subsidiary of Humana Inc., is looking for a primary care physician. You will work onsite at the Conviva clinic in Plantation, FL and report to the associate medical director. This role offers a team-based care model focused on whole-person care, more time with patients due to a lower daily patient volume, and a supportive, inclusive culture. Benefits include PTO, minimal call responsibilities, and dedicated CME time.
Requirements
- Doctor of Medicine or Osteopathic Medicine
- Board certification or board eligible in family medicine, internal medicine, or geriatric medicine
- Valid, unrestricted license in the state of Florida (or willing to obtain)
- Current unrestricted DEA license, or must obtain
- Two years' or more experience in primary care, geriatric care, or managing chronic conditions
- Must be screened for TB if selected for this role.

- Bilingual in Spanish and English a plus
Responsibilities
- Deliver comprehensive outpatient care to adult and senior patients.
- Maintain accurate medical records and documentation.
- Diagnose and manage moderately to complex medical conditions.
- Work with interdisciplinary teams to ensure holistic care.
