About The Position
Conviva Primary Care, a subsidiary of Humana Inc., is looking for a primary care physician to join their team. This role involves working onsite at the Conviva clinic in Pembroke Pines, FL, and reporting to the associate medical director. The position operates on a Monday-Friday schedule from 8:00 am to 5:00 pm. The role emphasizes a team-based care model focused on whole-person care, allowing for more time with patients due to a lower daily patient volume, and fostering deeper relationships. Requirements
- Doctor of Medicine or Osteopathic Medicine
- Board certification or board eligible in family medicine, internal medicine, or geriatric medicine
- Valid, unrestricted license in the state of Florida (or willing to obtain)
- Current unrestricted DEA license, or must obtain
- Two years' or more experience in primary care, geriatric care, or managing chronic conditions
- Must be screened for TB if selected for this role.

Responsibilities
- Deliver comprehensive outpatient care to adult and senior patients.
- Maintain accurate medical records and documentation.
- Diagnose and manage moderately to complex medical conditions.
- Work with interdisciplinary teams to ensure holistic care.
