Primary Care Physician (MD/DO)

Town Square Health•Rochester, NY
•$210,000 - $230,000•Onsite

About The Position

Town Square Health is hiring a Primary Care Physician (MD/DO) for its clinic-based, in-person West Ridge Road location in Rochester, New York. The physician will provide comprehensive primary care to Medicare-eligible patients in a value-based care model designed around long-term relationships, quality, and patient outcomes rather than visit volume. Patient panels are smaller than the national average, and visits are 20 to 40 minutes depending on patient needs. The physician will own the care of a patient panel and lead a team-based model, working with two dedicated Navigators per provider. Navigators provide expanded medical-assistant support and partner with patients between visits, including through AI-enabled workflows. The physician will also coordinate with specialty colleagues, including during simultaneous visits, and use tools such as AI scribes. Joining at an early stage, the physician will have the opportunity to influence the clinical workflows, culture, and care model for this clinic and future Town Square Health locations. The role reports to the Senior Medical Director.
