About The Position

Humana’s Primary Care Organization provides senior-focused, value-based care through CenterWell and Conviva centers. The Primary Care Physician at CenterWell Senior Primary Care delivers comprehensive primary care to seniors as part of a collaborative interdisciplinary team that includes advanced practice providers, pharmacists, care coaches, behavioral health specialists, and others. The role reports to the Associate Medical Director and is focused entirely on clinical work, including patient-facing care and general administrative time. The physician works on-site in an outpatient office setting and participates in a limited on-call rotation.

Requirements

• Graduate of an accredited MD or DO program from an accredited university.
• Board certified or board eligible (ABMS or AOA) in Family Medicine, Internal Medicine, or Geriatric Medicine.
• Hold a current, unrestricted medical license or be willing to obtain a medical license in the state of practice.
• Demonstrate strong interpersonal and communication skills with colleagues and patients.

Nice To Haves

• Active, unrestricted DEA license.
• Medicare and Medicaid Provider Number.
• At least 2 years of experience.
• At least 2 years of experience with Medicare Advantage patients.
• Knowledge of Medicare standards.
• Knowledge of HEDIS quality indicators.

Responsibilities

• Provide comprehensive primary care to seniors.
• Collaborate daily with the interdisciplinary team.
• Document in the Athena EMR.
• Participate in a limited on-call rotation.
• Spend all working time on clinical duties, including patient-facing care and general administrative time.
