About The Position
Hartford HealthCare at Home is seeking a Primary Care Nurse (Registered Nurse) to provide personalized care to patients in their homes. The role offers autonomy and is supported by the organization’s Person-Centered Care Model and technology for delivering and documenting quality care. The nurse works with patients and an interdisciplinary team to support individualized care, education, self-advocacy, and independence.
Requirements
- Registered Nurse with a valid Connecticut RN license.
- At least one year of RN experience.
- Valid driver’s license and reliable transportation.
- Computer proficiency.
